VBAC - 2/24/2010 6:12:21 PM

Since my baby boy decided he wanted to come into the world rear first (was full breech), I had to get a c-section. I was really disappointed that I couldn't at least labor naturally and that I may be stuck now with c-sections moving forward.

My doctor is one of the few in this area that will even consider a VBAC. According to him, I'm not a poor candidate for one, but I'm not a great candidate for one either since I had a section during my first childbirth.

So, I'm looking for some reassurance about VBAC's. I'm really nervous about my incision because it seemed it is taking a REALLY long time to heal. Any VBAC stories out there? How did it go? Were there any complications?

I had an emergency C-section with my first child (foetal distress).

My second pregnancy went very smoothly. My consultant examined my scar briefly at every visit, just to make sure it was doing ok. He was more than happy for me to try to birth vaginally, but recommended I had epidural anaesthesia in case I ended up needing another section. I was happy about that anyway because I'd had pethidine (demerol) for pain relief in my first pregnancy and had not reacted well to it at all.

Anyway, I went into on-off labour at home, and stayed home for two days (with a TENS machine for pain relief), then went in on the third day when the contractions were constant and regular. They set up the epidural, regularly checking my scar for any signs of possible rupture. When the time came to push, I was exhausted (through lack of sleep over the past 3 days) and needed a little instrumental help (Ventouse). Which involved me having an episiotomy and 4 stitches.

So, no complications, and everything fine and dandy. I would say that my post-partum for my VBAC was more uncomfortable in the short-term, but my recovery overall was much quicker.

4 years.

(We didn't have sex for the first two years after my first pregnancy because I was so terrified of getting pregnant again)

Im gonna pop in here, as I am curious about this, though I am one of the woman that cannot VBAC it even if I wanted to due to the way they cut my uterus. :(

Did you have a vertical incision? (sorry if I already knew, but forgot)

Jamie,

Did you have a "low vertical" incision, or a "classic" incision (which is also vertical but higher up)?

I would really like to have a VBAC this time around. . .I had a c-section with my daughter 2 years ago (1st child). She was breech too It was a low horizontal incision and healed well, so I was told I would be a good candidate. I'm really hoping for the best. One complication is that we're going to moving when I'm about 6 months along, so I'll have to find a new doctor and hope I can find someone willing to let me try a VBAC.

I don't know enough about it to know which one that is.

It's just that in some instances, they will let you try for a VBAC if you've had the former (though not the latter).

Hmmm. . . thanks for the link Manda! That was helpful.

From what I read it looks pretty favorable that I should be able to do a VBAC

Sometimes it has more to do with the hospital you are in verses the doctor that you have. My doctor is the odd duck of OB's at his practice. So hopefully we won't have to switch doctors for the next baby (if there is one).

Emily, do you know where you are delivering at yet?

If so, contact someone at that hosp and ask their policy on VBAC. It may be moot if they don't allow it. If they DO find out an OB from them that has privileges and supports VBAC. That would be my first step.

Our L&D told us at childbirth class, that only "larger" hospitals that have a full time anesthesiologist on staff will even consider VBAC in the case that an emergency C is neccessary. So in our state that means only the hospitals down in Little Rock are technically equipped to allow it. A local lady I knew wanted a VBAC so she found a doc in LR (2 hours from here) and at about 37 weeks she left her other kids with her parents and "moved" to LR to be there when it was time to deliver.

The thought of a hospital not having a full time anesthesiologist is so weird to me It really just depends on the malpractice insurance in your state, as to whether or not your doctor/hospital will allow VBACS.

ICAN is a great VBAC resource.

I was just going to reference ICAN, thanks for doing so Ryanne. If your OB isn't willing to do one you could always find a midwife who will assist you with a homebirth too.

Gotta love tiny towns. Our hospitals have only been doing EPIDURALS for about a year and a half now...that was big news when they started THAT. Oh and we have ONE OB. haha Nothing any bigger though for about 100 miles.

Well that's where the difficulty lies. We are going to be commissioned as Salvation Army officers in June and we don't find out where we will be going until then. It will be somewhere in a 10 state area of the mid-west, that's all we know.

The hospital where I had my VBAC didn't have a full-time anaesthetist; it had a part-time one who was on call for the rest of the time. All the hospital told me to do was phone when I was coming in, and then they'd call him, so that by the time I was there and booked in, he'd be there, available.

My hospital is definitely a community hospital...not large by any means. We don't even have anesthesiologists...we have CRNA's. If a mom comes to our hospital and is having a VBAC, they call the entire OR crew in and they have to be at the hospital the entire time the mom is in labor. If that means the entire OR crew is sleeping at the hospital overnight "just in case" then that's what they do.
Love my hospital!

LOL....the hospital in our town has 11 beds. No anesthesiologists either. But, they also don't deliver babies unless it is a true emergency. That is why I had to go to the big city to find an OB.

A good friend of mine has had two VBACs since her first (emergency c-section, can't remember the reason). I am not sure about her hospital's attitude, but when she told her doctor she wanted a VBAC, he was surprised but supportive. He did schedule a c-section sometime shortly after her due date in both cases since you can't be induced once you've had a c-section, but I think that made her all the more determined to get her babies to come on time. Both of her natural labours went very well. Probably has something to do with her being in amazing shape (she was teaching 4-5 weight-lifting classes at the gym right up until she was due). I'm not sure if she used any meds for pain relief.

Come to think of it, I have another friend that has had two VBACs since a c-section; in her case she had a scheduled c-section with her second child (full placenta previa). She was with a midwife for all her pregnancies; she even stayed with my friend for the c-section even though she had to transfer care. The first VBAC was in hospital and the second was a home birth. I got the impression her midwife would have been supportive of a home birth with either, but that the hospital with the first VBAC was my friend's choice.

I've gotten the impression that Canada's attitude towards C-sections and VBAC is a bit different though; I believe our c-section rate is lower, for example. Midwifery is also set up differently.

It's not that much different actually. The US c-section rate is 1 in 3. For some reason all I could find was articles from 2004 for the Canadian rate which was from 2001-2002 at 22.5% or roughly 1 in 5 births. That is a jump from only 15% from 1979-80. But again that data is over 8 years old.
